I wanted to know when I will get a new SS card and the return of submitted ID documents but it was difficult to get any information. My SS form was sent to you on 09-09-19 by USPS.
I just wanted to say that the people who work in this office are for the most part very polite. When I have had issues with my state office they have usually been able to resolve it. A big thank you.

I changed my name recently by court order. I have several copies of my name change order. I filled out SS-5, provided a copy of the order and provided my medicaid card. It was dropped off in a manilla envelope at the office on 16241 N Tatum Blvd. I received a letter yesterday saying I didn’t provide enough documentation to show my identity. Which is a crock. On page 2 of the SS-5 form, a medicaid card is listed as an acceptable card for identification purposes. My name change order also has all the information they needed. I did everything I was supposed to do. I was also treated very rudely over the telephone. I could have faxed this stuff in, but nobody mentioned that as a possibility.
