I just called the Duluth Office of Social Security on 130 West Superior Street-Suite 400. I am expecting a call back in less than a half hour. I found my part A card--but I can't see the Part B card. I need to get a replacement Part B card ASAP. I have to send it back to the place I got a form letter from the first part of June. I am not doing Part B--I was not informed of the penalty of 10% per year for not doing Part B. I called the end of last year and was not told anything about the penalty and also that it goes up 10% every year if I don't have Part B. I was told the amount to be deducted was $104.90. I had called one of the head offices first last year--they never said a word about the penalty either!! Never was I told about the penalty. Therefore I need a replacement Part B card so I can send it back to Mid-America Service Program in Missouri before the end of this month. I am not going to have $167.80 taken out of my check!! I guess I will just do my best to stay healthy!! Hopefully I can do so!! I was going to appeal but I don't see that happening within the next month or so!! So I won't be doing that either!! I just wanted to write this also besides getting the phone call. I appreciate the time and help!! Thank you Eloise Christiansen
