I've needed to finish a legal name change for many months. They've made it impossible to do this, and now other services are having problems because my name does not match my social security number.
I am a 68 year old retired senior on Social Security. I was retired on a disability due to injuries and I'm going to multiple surgeries at this time I have been for several years. I am wondering if I need to make any changes to my Social Security plan and if I'm able for additional health Care or Medicare benefits or Medicaid. I would like to come in with a meeting as soon as possible because I have surgery on my foot on the 21st and I'm having continued problems with a brain tumor that I have. I am now living with my son in Portland and he can help me make appointments on any Friday. Thank you

I am very displeased that the Portland, OR., 17925 SE Division
St., office misplaced my birth certificate and W2 forms which I was instructed by an SSA representative to mail to this office to complete my online application for SS retirement. After 2 phone calls to the person in this office who should have received my letter with docs enclosed I was most displeased that she reported that my letter never reached her desk. She sent an email to the other reps in this office asking if it reached them in error and her report to me upon my second phone call was that no one acknowledged having my letter. I can only believe that the process of sorting and distributing mail in this office was handled carelessly and that my most important identification documents have been misplaced. I requested in my letter that they be returned to my home address, which I also provided in the letter. It is still my deepest hope that the docs be located and returned to me.
