Overall rating of this office:I visited the office on 11/17/17 to apply for Medicare. I met with Mrs. Ballesteros who discovered that the employment verification form was incomplete. She tried to call the HR person at my company. However, the HR person was not at work that day. Beginning yesterday, our HR person has called at least 3 times for Mrs. Ballesteros without a return call. I am concerned that Medicare will be in place by 1/1/18. I would appreciate it if Mrs. Ballesteros could call the HR person, JoAnn Siekas, today to resolve this matter. Thank you.

this office is not operational - when I arrived the doors had big red bolts - this office is not open but closed.

I have not yet received my INITIAN medicare card as yet, for Medicare Part A.
